Create an order page for a business that sells one product. Customers (users) will enter their name, their shipping address, their phone number, select the quantity of the product, and select a delivery method. Form must be neat and styled functionally. Include a picture and description of your ‘product’. All data must be validated and sanitized. Order data should be saved to a database. Your order form must use at least one of each of the following: text fields, radio buttons, drop down lists, and checkboxes (you can add additional questions to meet these requirements).

Your form must be sticky. That is, if a user fails to enter in valid data in one or more fields, the user will have their previous entered data presented to them. 

This form should use single page processing/be all-in-one. Use POST.

You must validate all entered data.

Specifically, names should be letters, spaces, and apostrophes only, addresses should be broken into appropriate parts and check for validity (for example, zip code should be 5 digits; also, assume all addresses are US addresses), phone number should be in the format you request with appropriate instructions to the users, quantity should be a number, and delivery type should be one of your valid delivery types. Validation should always be done on the server side and also on the client side where applicable. Error messages should be displayed to the user in a way that is functional and useful; specifically, errors should be displayed to the user and instructions on what they need to do in order to correct it.

Create a simple database to store the orders. You may design your database as you wish, but it should be rather simple and make sense for the application (that may mean only one table).

Create summary page which will display all the orders neatly in a table. Display the data in three times in three separate, labeled tables, ordering by last order placed first, first order placed first, and highest quantity ordered by customer first.
