INFO 4360 PROJECT 2 TEXT ANALYSIS WITH SPSS MODELER In this project we are going to conduct word extraction, category analysis, and sentiment analysis on a training data set, and we are going to build a dictionary that we can apply to a test data set to determine if our dictionary can be used to generate accurate sentiment analyses. In both cases (training and test) we have the advantage of numerical ratings of our topics, so we can properly train our dictionary, and we can also test the performance against our test data. We will use the data set: INFO4360DataDoctorRatings. Our training data set will be Denver and our test data will be San Diego. We will build dictionaries for and test against the four categories: Staff, Punctual, Helpful, and Knowledge. The goal is to produce a dictionary that can be used by the American Medical Association (AMA) to evaluate doctors on these four categories based on patient comments alone (in the absence of any numerical or objective categorical ratings). Deliverable. The deliverable for this project is a business report, written for the AMA, that presents your findings. OPTIONAL REMARKS. Everything above contains all you need to complete the project. However, in the case that you might want a little more guidance, you are invited to keep reading. 1. You have a number of SPSS outputs to consider (concepts, categories, clusters, word associations, types, etc.). Which ones do you intend to use? (Ensure you explain to the AMA what you used, and why.) 2. SPSS has some built-in resource templates that allow you to perform quick analysis. You should probably document any modifications you make to these. 3. This project has four requirements (Staff, Punctual, Helpful, and Knowledge). Each should have its own dictionary and analysis. 4. From Project 1 you should understand how you can assign values to words, and how you can use those words to “predict” measures that you already have (in this case, numerical ratings). You can also test your predictions against the original values. However, this problem adds another step. How are you going to determine if the words (and their values) are good predictors? This analysis, which should be detailed prominently in your report (most likely in appendices) should form the basis for your dictionary creation. (Of course, the ultimate measure will be the actual performance.) 5. If you come up with dictionaries that are effective at predicting, ensure you highlight your success in the report, but certainly detail your process so that the work can be duplicated. If you find that the methodology fails and that you are unable to produce an effective predictive dictionary, then your report just got a lot longer. (It is much more difficult to show that nothing works than to show that something works.)

He kept asking me if I had been raped (he asked 4x) or had physical abuse, emotional abuse, etc. to the point of creepiness. Then he diagnosed me with all other problems, depression when I told him I was in a good place except for physically. Then he said I had to take care of all of these supposed issues before he would write me a script. I think he lost his ability to write one so he strung me out with this bs of weird issues like flashbacks so he take my $375 and then shut me down, unconscionable! The office staff is disorganized and unfriendly. They lost my insurance card on one occasion and were forced to rummage through seemingly countless piles of paperwork on various desks and tables before finding it. Despite arriving 10 minutes early for every appointment, I always had to wait about an hour to see the doctor. Dr. Gordon himself was brusque, but I thought he was thorough and genuine. The last time I scheduled an appointment, I was called back to a different waiting room. Dr. Gordon came in and said he wouldn't see me because I'd missed to many appointments and that was "disrespectful". The fact is, I'd NEVER missed an appointment. He talked so much about his family that the report he was typing and later sent me, contained another patients info. I have been under his treatment for over 8 months and still have the same hormonal issue I started with. He has changed my perscriptions 5 times. One perscription took his office staff 1 month to call into my pharmacy. After several attempts via email to get things straightened out, he has cut off all email and phone communication with me and insists that I make another appt, at a cost of over $300. During the appts, he asks the same questions he asked you during the last visit and types it on his computer. In my case, the answers are the same as the last visit since there has been no improvement, so his motives don't seem to lean towards helping me with my symptoms. Chambers, E. Dr. Chambers does not really seem to care about the patient or what kind of pain you're in. He just churns and burns through each patient.. you can wait an hour just to see him for less than 5 minutes. If you're in pain, don't expect any kind of realistic medication (I have a back injury from a motor vehicle accident and haven't been able to sleep properly for months. He does not care!!!) The first time, she correctly diagnosed my eye allergy, but then proceeded to insist on giving me a breast exam (to pad the insurance bill, I assume) and tell me that I needed to get off birth control (HELL NO!). Before my appointment today, I was told that I had to sign a waiver protecting her from malpractice. I declined to sign and was told "all doctors do this" by her rude receptionist. I then asked where the nearest Urgent Care was and was told "I don't know, but you can't go there unless Dr. Levine refers you." Two lies in a row was enough. I brought in documentation from a trip to the Emergency room with possible thyroid issues. She actually said to me that there were doctors in her office who had thyroid issues and that they were taking a certain medication that helped them IMMENSELY. When you're sick, that's what you want to hear. Did not prescribe. Unkind to get a patient's hopes up. I was sent to get blood tested two or three times to have her NOT DIAGNOSE ANYTHING and to get a PSYCH EVALUATION BEFORE SHE WOULD "let me leave". As it turned out, I had a SEVERE CASE OF MONONUCLEOSIS which was causing panic and anxiety attacks and issues with my thyroid. I was depressed because my activity level was so low. I was left waiting in the examining room for 50 minutes with no communication from staff. When I tried to schedule follow up appointment he made it for 2 weeks out and then refused to return my calls. I requested my medical records be forwarded to another physician and my request went ignored. I finally went to the office to request in person and Dr. Wickes verbally attacked me and told me I was not a priority and my request was not important. He would forward my records in 3 weeks if he got to it. Then told me to leave his office because I was tresspassing. I took a co/worker who witnessed this unacceptable and unprofessional behavior. Over the course of a year, my father was losing weight, had GI irregularity, and other classic cancer symptoms. Doctor Saavedra prescribed Pepto Bismol for my father's cancer symptoms for many months. By the time my father was correctly diagnosed by a different doctor, he had less than 2 months to live and say good-bye to his wife and 3 school-age children. Could not treat my diagnosis however kept having me come back over and over and over, rather than refer me to the correct specialist. Wrong diagnosis and sarcastic attitude - I have a knee injury but he ignored the injury, would not treat said "your knee is not going to be like it was when you were 17" Duh! - said it was gout, turns out to be much more serious and now can not be corrected due to lack of treatment.....Stongley recommend you do not see this Dr if you want to get better...see him if you want to go untreated with possible negative results due to his lack of knowledge about serious problems. His answer to most all problems, is to prescribe another pill! I Found a Dr who knows what he is doing. I have a torn PCL and torn Meniscus. spent a lot of time in tremendous pain…..As a result I now have long term injury that can not be corrected.... She has been excellent and concerned for my well being through out the management of my care with various other specialists and setting up my referrals. When I have an appointment with her, I get to converse on an intelligent level and exchange of thoughts with her. Yes she has her laptop as if glued to the hip, but I don't mind, because it means to me that she is just being thorough with my medical history. The staff could be a little more personable but I'm not there to see them. Drug prescription refills have been an issue and I took it up with Dr Eros and she said to call & talk direct to her, so I will try it next time. Over all I am quite pleased with her and like her! Dr hamshire's knowledge and treatment of my cancer was what you would expect from a Dr of his credentials and knowledge... I am happy to say dr Hampshire and the cancer ward nursing staff treated me like I was a king. All question were answered. all requested treatment was done promptly. Total trime for treatment 50 Days and 5 of thoses were in the ICU. It was touch and go for me, but with the staff and doctor I never had a doubt; I would survive. He does do things differently than what you've done before. If this is not your first stop than that's probably a good thing because what you did before obviously didn't work. It is easy to point fingers of blame but sometimes you need to look in the mirror too. Bay recovery is good, relaxed, you get out what you put in. I don't mind waiting 3 hours to see Dr Rand. I expect it so I bring a book. The programs, education and treatment are there. Biggest problem - The support staff employees are not as good as Dr Rand. He was screaming at me in front of my wife just for asking to return our phone calls in a timely manner. His hrs of operation hard to figure out - he is barely in the office during promised 9-5pm and almost never returns phone calls. Second time I saw him, while talking to his receptionist, he started to scream at me and when I asked why, he threatened to call police... He should never see another patient again. I had to recently move from San Diego and if there was some way that I could afford to fly back and see her for my quarterly visits (I'm a diabetic), I would. She was concerned about not just my health, but also about my overall quality of life and care. Not only did she accuse me of going off my medication on my own, and argued with me about it, until she checked my file and saw that she took me off (too soon), she slammed my file on the table and threw a fit, yelling at me, and using scare tactics on me. Two appointments later she read my blood results wrong (even I knew she was mistaken), and told me I was getting worse, when, in fact I was was improving. She told me I was on the verge of going into Thyroid Storm. I left very upset and stressed, knowing she was wrong, and feeling helpless because I have no insurance, and had to rely on her for my care because I can't afford to go anywhere but the Family Health Center. I got three other professional opinions, with the help of friends in my community, and she was mistaken. I was improving. He is an expert in anti-aging medicine and is very aggressive in encouraging his patients to go outside of their insurance coverage to visit him on Wednesdays, which he sets aside for special consultations. I went to a couple of them and got his comprehensive health evaluation, which was merely a restatement of my health issues and his thoughts on what might be causing them. In order for me to obtain actual treatment for the issues, however, I was advised to continue to make the Wednesday appointments to the detriment of my pocketbook. His office staff is wonderful, though. I'm switching doctors after having been with him for many years because he seems less interested in finding out what's causing my problems than in prescribing homeopathic medication out of his office that I'm sure he makes a profit on. Both of his dxes required me to take prescriptions long term, one was for life. I saw a specialist for each of the dxes Dr. Considine had given me. Both specialists over-ruled Dr. Condsidine's dxes. I no longer take either of the medications Dr. Considine prescribed me. By far, the thing I'm most displeased with was his insistence that I need antidepressants for my complaints of fatigue, joint pain/tingling and resulting concern over the persistant nature these conditions. HOWEVER, Beliveing more in myself than in Dr. Considine, I went to another Dr. for a second opinion. My new Dr. recommended I see a Rheumatologist. I did. She dxed me with an ongoing viral infection. He is always sure to provide the medical information necessary to make an informed decision and does not push procedures or medications on anyone. His practice is faith based (Catholic) and he does not refer for abortions, sterilization, in-vitro, etc. I admire that he does not sway from his personal beliefs regardless of whether or not I agree. His staff is friendly and he has 2 offices and a nurse practicioner on staff. Offers natural family planning, pediatric/OB care, and general practice. Refers for spiritual counseling. Nicholson, Laura J. 6/18/13 SanDiego 5 3 1 1 Dr. Laura Nicholson treated my dad in the ER at Scripps Memorial hospital for atrial fibrillation and fluid in the lungs. Her bedside manner was insensitive, inappropriate and offensive. She bluntly told my dad, who is a kidney transplant patient, that there was no way he would ever get a kidney and even if he did he would code on the table. Not only was this information incorrect but extremely damaging to my dad’s morale. Being in the ER and in his medical state, my dad was already scared, anxious and uncomfortable. Dr. Laura Nicholson not only destroyed his confidence but also caused him emotional harm. We asked for another doctor and reported this incident to the hospital. The hospital contacted her regarding our complaint and a few days later she left our family a voicemail. Her message was unapologetic, insincere, and agitated. I was one of her first patients out of med school. My last physical was all of 15 mins. long and she told me to " make another appt. if I wanted to talk longer." Physical was a pap and breast exam. Asked about menopause told me that she isn't interested in blood tests just " goes on symptoms." This woman is unsmiling and unhelpful. She is not a good doctor. I feel as if I am boring her when I see her. Thank goodness we can change doctors. Personality and empathy of a stick. Leung, Jennifer 6/25/13 SanDiego 3 1 3 1 Dr. Leung was friendly with me however misdiagnosed a malignant melanoma mole. She made me wait for a month until she had open time on her schedule. By the time I found out I had cancer, it spread to my lymph node. It isn't her fault, I had cancer, I don't blame her. However, look for a doctor who will be precautionary as it can save your life. I remember her telling me specifically that the mole did not look suspicious, however it was completely different than any other mole I had. I also told her that it itched, which now I know is a warning sign but she ignored two basic common warning signs. If you chose to see her, please be proactive about your health, as she will not be. Look, Michelle L. 9/28/11 SanDiego 4 2 1 1 Dr. Look is not a responsible doctor. She prescribed me Savella last year to repalce my anti-depressant, even though Savela is not an anti-depressant. She advised me not worry about it, to continue using it. After two weeks of the worst depressive episode ever in my life, I told her I needed to be put on the old anti-depressant. She argued, saying I was bi-polar and advised me to see a shrink for a proper diagnosis. After I declined, she contacted my husband at work, advised him I was bi-polar, a physical threat to myself and refusing care. My husband asked me to see the shrink she referred and after a half an hour with the shrink, was told I suffer mild depression but was not bi-polar. The shrink also questioned why I was on Savella. She stopped caring about treating me, broke my trust by going behind my back, upset my husband and our life and worse of all, the Savella could have killed me. Matsumoto, Paul M. 7/12/07 SanDiego 5 3 2 2 Dr. Matsumoto is a pleasant man and was my PCP for nearly three years. He was responsive to prescription requests and followed-up on my blood pressure. Beyond that, I never felt like we connected, because I had to give him my medical history at every visit. It's been my experience that the doctor does a quick chart review before entering the room in order to avoid this problem. I'm not saying that he didn't do this, but if he did, it was never evident to me in our interactions. When I brought him info from the Sharp website and requested a referral to the pain specialist and pain program, he did refer me to PT, but he refused the pain specialist consult as not appropriate. Fast forward, two years: my current pain specialists don't understand why I wasn't referred to the pain specialist since she provides the exact services that I needed. She even refers some of her patients to the clinic I am now attending! Lost two extra years to pain, not a happy patient here. I have worked in the healthcare industry for over 30 years and usually the discontent are those that don't get the drugs they want. Dr. Noble takes time with each patient, but that doesn't make up for the rest of it. Most DO's appreciate patient input and trust their patients to know their bodies, not so in this office. Good because you feel he's listening to you and you're getting personalized attention to your specific needs, but bad because this often makes wait times longer than usual. However his staff is another story. They've been pleasant enough about half the time, but the other half they've been very rude and cold which is unfortunate because he's such a great doctor. He's also willing to incorporate homeopathic remedies with traditional western medicine which is not usual for MDs. If you're willing to wait and deal with a sometimes rude staff though, I highly recommend him. His staff is the main reason for the not stellar rating. Both of my kids have special needs and she is very good with them. The appt wait time at this office tends to run 30-60 min past your appt but that's true with ALL the dr's there not just her. If she is not in then you see another dr in the practice and honestly I've been happy with them all. I would recommend this practice to a friend. First instead of realizing that she had appendicitis kept her without a proper diagnosis until her appendix ruptured and even when it was ruptured and she had high fever, stomach pain, etc he kept telling us that it must be an infection. Needless to say, after 2 weeks of her ruptured appendix another doctor realized what had happened. She was in great danger for 6 weeks (The worst 6 weeks of my life) then after her surgery and the placing of a midline for the antibiotic she developed a blood clot in her arm that again Dr. Schlitt said the bruise on her arm was nothing to worry about and that same week it was discovered by another doctor that she had developed a blood clot and now she is taking blood thinners for 3 to 6 months. Also in these six weeks he never bothered to call her to see how she was doing inspite the fact that he belongs to Scripps and thus able to see her record online. His staff are courteous and polite as well. He is by far the best doctor my family has had and I have recommended him to many people looking for an MD within the Scripps system. The HMO world makes it difficult for physicians to actually have bedside manner. Dr. Slater has this. I appreciate all that he has done for my son and myself. I was their for my issue and the only thing she seemed to care about was my "PAP Smear" and why I hadn't gotten it yet. (I got it at a different facility). She was rude about it. She told me she'd call me back with my lab results. About 3 1/2 weeks later I received a standard letter in the mail stating my results "came back normal." Nothing else. It didn't say whether or not to follow up with her or my Allergist. So meanwhile, it's been about 8 months with no relief from what I am suffering with. Also, I was the first patient of the day and I ended up having to wait about a half hour. Are you kidding me? It's not like she had a patient before me!!! Trust me...choose someone else. He tries to be friendly and amiable, but quickly gets down to business. I felt like he didn't really address my concerns and didn't really care in general. He seems to just prescribe pills to his patients. I overheard him say the exact same phrase to a patient in another room while I was waiting: "I'm going to put you on so and so...[pills]". I explicitly told him I don't like pills, and he said that no one does. That was the end of it. I'm not happy with him. He (and I guess doctors in general) charges too much, spend too little time with the patient, and doesn't seem to care. I hate going to the doctor, because it seems like I always pay a lot of money and I never feel any better afterwards.I would not recommend him.
